Physical Therapist (PT) - Long-Term Care
LP Therapy
LP Therapy is seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function.
Responsibilities- Review physician referrals and medical records.
- Evaluate strength, mobility, and functional ability.
- Develop and modify treatment plans.
- Administer therapeutic exercises and modalities.
- Educate patients and families.
- Complete timely Medicare documentation.
- Collaborate with care teams.
- Supervise PTAs and aides as required.
- Support quality outcomes and productivity standards.
- Bachelor's degree in Physical Therapy.
- Current state PT license.
- LTC experience preferred.
- Strong knowledge of Medicare guidelines.
- Effective communication and teamwork skills.
LTC facility; physical demands include lifting and mobility assistance.
