Job Summary

The Home Care Licensed Practical Nurse provides direct nursing care as prescribed by the physician to acutely or chronically ill patients in their homes. Responsibilities include recording clinical notes and coordinating patient care activities to ensure high-quality care within the assigned geographic area. The nurse works under the supervision of a Registered Nurse or designated supervisor, adhering to the State LPN/LVN Practice Act, licensure requirements, and agency policies.

Specific Job Duties/Responsibilities

• Provides clinical services within the scope of practice, following the plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record, complying with regulatory and payer requirements.
• Participates in care coordination, reporting patient needs and changes, attending case conferences, and communicating effectively with patients, families, physicians, and team members.
• Maintains confidentiality of patient information per agency policy.

License Requirements

• Current LPN licensure in Louisiana without restrictions.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Current CPR certification.

Experience Requirements

• Previous home care experience preferred.

Additional State-Specific Requirements

• At minimum, one year of experience as an LPN in Louisiana.
