Job SummaryOdyssey House Louisiana, Inc. is seeking a full-time Registered Nurse for the Community Health Center. The schedule is Monday‑Friday 8am‑5pm. This role is a full‑time grant funded position by the Hepatitis C Elimination Initiative Pilot. The Registered Nurse (RN) provides and coordinates patient care for participants in the agency’s Hepatitis C (HCV) grant program within the Federally Qualified Health Center. This role offers direct clinical support, including monitoring patient progress, facilitating access to HCV treatment, and providing education on prevention, treatment options, and overall liver health.
Reporting to the Nurse Manager, the RN is responsible for developing and implementing individualized care plans in collaboration with physicians, social workers, and other healthcare professionals to ensure seamless, high‑quality care. The RN also serves as a patient advocate, helping participants navigate barriers to treatment, empowering them to make informed health decisions, and supporting them throughout their HCV care journey.
- Assesses patients' conditions, monitors vital signs, administers medications and treatments, and ensures patient safety.
- Develops and implements individualized care plans, collaborating with other healthcare professionals to ensure comprehensive and effective patient care.
- Educates patients and their families about health conditions, treatment options, and preventative care. Also advocates for patients' needs and ensures their voices are heard.
- Works closely with physicians, therapists, social workers, and other healthcare professionals to coordinate patient care and ensure a smooth transition of care.
- Administers medications, including injections, IVs, and oral medications, ensuring accuracy and safety.
- Monitors patients' responses to treatment, assesses their progress, and adjusts care plans as needed.
- Prepares to respond to emergencies, providing immediate care and support to patients in critical situations.
- Stays up to date on the latest advancements in healthcare, participates in continuing education, and maintains professional certifications.
- Documents all pertinent health-related information about clients on the patient chart that is done routinely and according to nursing standards of practice.
- Completes all incident reports before the end of the scheduled shift if applicable.
- Maintains HIPAA guidelines.
- Performs other duties as assigned.
- CPR certification, first aid training, and experience in specific areas like emergency or critical care nursing helpful.
- Ability to work collaboratively in a multidisciplinary setting.
- Proficient in electronic health records.
- Associate's Degree in Nursing, Bachelor of Science in Nursing, Nursing Diploma or equivalent.
