The Vietnamese‑speaking
Family Nurse Practitioner (FNP) will provide comprehensive primary care services to patients across the lifespan in an outpatient setting, with a strong emphasis on serving Vietnamese‑speaking patients. The FNP will conduct clinical visits in both Vietnamese and English, support chronic disease management, and participate in team‑based, integrated care. This position is ideal for a clinician who wants to combine clinical practice with meaningful community impact.
Direct Patient Care
Provide evaluation, diagnosis, and treatment for acute and chronic conditions for pediatric, adult, and geriatric patients within the FNP scope of practice.
Perform comprehensive history and physical exams, develop differential diagnoses, and create evidence‑based treatment plans.
Conduct visits in Vietnamese and English; use interpreters for other languages as needed.
Prescribe medications, order and interpret labs and diagnostic tests, and provide follow‑up care.
Manage common chronic diseases (e.g., diabetes, hypertension, hyperlipidemia, COPD/asthma, hepatitis, depression/anxiety) with a population‑health lens.
Promote preventive care including immunizations, cancer screenings, and wellness visits.
Provide culturally appropriate counseling on lifestyle, nutrition, medication adherence, and self‑management.
Collaborate with physicians, PAs, nurses, behavioral health providers, Care coordinators, and MAs to deliver coordinated care.
Participate in huddles, case conferences, and quality improvement meetings.
Facilitate referrals to specialists, hospitals, and community resources; ensure appropriate follow‑up.
Document all encounters accurately and timely in the electronic health record (EHR), following FQHC and payer requirements.
Adhere to clinical guidelines, standing orders, and care protocols.
Participate in quality improvement initiatives, UDS and value‑based measures, and risk‑stratified care management efforts.
Support compliance with HRSA, state, and other regulatory standards.
Provide culturally and linguistically appropriate services for Vietnamese and other limited‑English‑proficiency patients.
Help develop or review patient education materials in Vietnamese.
Participate in community outreach events, health fairs, and educational sessions as needed, occasionally on evenings or weekends.
- Master's or Doctoral degree in Nursing from an accredited program.
- Current, unrestricted Family Nurse Practitioner license in the State of Louisiana (or ability to obtain prior to start).
- Current national board certification as an FNP (AANP or ANCC).
- Valid DEA registration (or eligibility to obtain) for Louisiana practice.
- Current BLS certification.
- Demonstrated Vietnamese language skills (conversational or fluent) sufficient to conduct patient visits and provide education directly to patients and families.
- One or more years of experience in primary care, family medicine, internal medicine, or community health (new graduates with strong community background considered).
Primarily weekday clinic hours; occasional evenings or weekends for community events or extended hours as needed.
Outpatient clinic environment with standard exam rooms, integrated health services, and on‑site support services.
