A company is looking for a Case Manager Nurse- RN.
Key Responsibilities Conduct thorough assessments of patients' needs and develop treatment plans in collaboration with healthcare providers Monitor and evaluate the effectiveness of treatment plans and maintain communication with patients, families, and multidisciplinary teams Document case management activities and perform Utilization Review for assigned members while ensuring compliance with confidentiality policies
Required Qualifications Graduation from an accredited Registered Nursing (RN) program Current California RN license; multi-state license preferred Minimum of five years of medical/surgical or acute care experience, including two years in case management Prior experience in emergency room, critical care, or relevant clinical care Knowledge of medical claims and coding (ICD-10, CPT, HCPCS)
