* Job Description:

**** A Brief Overview
** Coordinates data collection for the Resident Assessment Instrument (RAI), including the MDS, CAAS and the Multidisciplinary Care Plan for long term care.
** What you will do
*** Completion of the MDS, CAAS and Care Plans are done by required completion dates and submit to the state of Iowa by submission date.
* Evaluate nursing and clinical intervention outcomes, conferring with other health care team members as necessary.
* Monitor resident medication usage and results.
* Participates in discharge planning and other necessary team conferences.
* Interview resident as designated and review documentation related to medical conditions, such as adverse reactions to medication or treatment, condition change, or progress or lack of progress with interventions.
* Work as part of a health care team to assess resident needs, plan and modify care, and implement interventions as appropriate.
* Demonstrates accurate keyboarding proficiency and computer navigation skills.
* Participate in activities aimed at professional growth and development including conferences or continuing education activities.
* Collaborates with assigned RN to refer residents requiring more specialized or complex treatment to psychiatrists, primary care physicians, or other medical specialists.
* Must have current knowledge of the entire RAI process and current regulatory requirements related to this process and implement.
** Qualifications
* ** Associate's Degree (AA) In nursing (ADN) Required
* Bachelor's Degree in Arts/Sciences (BA/BS) In nursing (BSN) Preferred
* 1-3 years Nursing home facility experience within the last three years Required
* 1-3 years Experience in the RAI process Preferred
* 1-3 years

Experience with Microsoft Suite (Word, Excel, PowerPoint, Teams, and Outlook) Preferred
* Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
* Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
* Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation.
* Being aware of others' reactions and understanding why they react as they do.
* Actively looking for ways to help people.
* Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.

* Registered Nurse Iowa Board of Nursing (IBON) (United States of America) Required
* Basic Cardiac Life Support & First Aid - American Heart Association within 60 Days Required
* Dependent Adult Abuse Mandatory Reporter - Iowa Department of Human Services within 180 Days Required
