Job Description
Serves as an integral part of the post‑acute care strategy to effectively reduce hospital admissions/readmissions and potentially preventable Emergency Department (ED) visits. Responsible and accountable for the coordination of care for transitional care (post‑discharge) patients. The Registered Nurse will assess and provide care as it relates to the specific needs of the patient within the framework of the State Nurse Practice Act.
ResponsibilitiesCoordinate transitional care for patients to reduce readmissions and ED visits. Assess patient needs and provide nursing care in accordance with the State Nurse Practice Act.
Minimum QualificationsGraduate of an accredited School of Nursing and three (3) years of experience in an acute clinical setting. Demonstrated strong organizational, decision‑making, delegating, communicating and customer‑relations skills. Current R.N. licensure with the Louisiana State Board of Nursing and current Basic Life Support (BLS) certification through an ILCOR‑approved course. ACLS and/or PALS is preferred.
- Must have a valid and current state‑issued driver’s license with reliable transportation and current safety inspection and license plate registration.
- Must maintain and provide proof of automobile insurance in amounts meeting at least the minimum liability coverage limits required by the state of residence.
- Cleared driving record check; records may not contain a suspended or revoked license, three or more moving violations in the past 36 months, two or more accidents in the past 36 months, one or more DUI/DWI within the past 24 months, being at fault in a fatal accident within the past 5 years, leaving the scene of an accident within the past 36 months, or reckless driving within the past 12 months.
Bachelor of Science degree in Nursing. Critical Care, Emergency Care, and/or Home Health experience is a plus.
Physical DemandsMust possess good physical health. Requirements include standing, sitting, or walking for long periods of time. Lifting at least 50 pounds is required. Ability to work with moderate noise levels.
