Patient Navigator Nurse
As a Patient Navigator Nurse, you serve as the hub of your patients’ multidisciplinary team, advocating, interpreting, coordinating, educating, supporting, and counseling patients throughout their care journey. You are the critical connection for patients recovering from strokes, cancer, and long‑term illnesses, ensuring seamless communication and optimal outcomes.
Responsibilities- Coordinate written and verbal communication regarding the medical treatment plan between the patient, family, physicians, physician staff, and other team members.
- Communicate and coordinate the treatment plan to internal and external departments and agencies, establishing and maintaining continuity of patient care.
- Assist patients in understanding their diagnosis, treatment options, and available resources.
- Review medical documentation to help physicians obtain complete and accurate records of the treatment plan.
- Serve as an essential link between patients and all other care providers.
- Follow patients throughout the care continuum, collaborating with care management resources and facilitating appointments for consults and support services including social workers, PT, OT, ST, dietitians, physicians, and financial resources.
- Collect data, track outcomes, and support strategic planning processes; plan, implement, and evaluate patient care from screening to survivorship.
- Assist patients and their families in resolving treatment‑related problems and making difficult decisions through education and support.
- Review and update policies related to services provided to enhance professional practice.
- Serve as a resource for community educational events such as health fairs, screenings, symposiums, and lectures, and for staff education.
- Current nursing license to practice in Louisiana as defined by the Louisiana State Board of Nursing.
- American Heart Association BLS certification.
- Minimum of 3 years of professional nursing experience in a specialty area.
