Job Summary

The Patient Navigator Nurse serves as the central hub for patients recovering from strokes, cancer, and long‑term illnesses, advocating and coordinating care across multidisciplinary teams.

• Coordinate written and verbal communication regarding the medical treatment plan among patient, family, physician, and other team members.
• Communicate and coordinate the treatment plan with internal and external departments and agencies to maintain continuity of patient care.
• Assist patients in understanding diagnoses, treatment options, and available resources.
• Review medical documentation to help physicians complete accurate treatment plans.
• Serve as an essential link between patients and all other care providers.
• Follow patients through the care continuum, collaborating with care management resources and facilitating appointments for consults and support services (social workers, PT, OT, ST, dietitians, physicians, financial resources, etc.).
• Collect data, track outcomes, and support strategic planning processes; provide nursing care and guidance from screening to survivorship.
• Assist patients and family members in finding solutions to treatment‑related problems, providing education, and supporting difficult decisions.
• Review and update policies related to services provided to enhance professional practice.
• Serve as a resource for community educational events and staff education.

• Valid nursing license to practice in Louisiana (as defined by the Louisiana State Board of Nursing).
• American Heart Association BLS certification.
• Minimum of 3 years of professional nursing experience in a specialty area.
