As the Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care as prescribed by the physician to acutely and/or chronically ill patients in their homes and record clinical and progress notes; coordinate care activities to deliver the highest quality of patient care in the geographic service area assigned. You will work under the direct supervision of the Registered Nurse or designated supervisor.

• Provide high‑quality clinical services within the scope of practice, in accordance with state laws and the plan of care, and coordinate with other health‑care team members while following infection control procedures.
• Document patient visits thoroughly in the electronic medical record using the agency’s electronic visit verification program.
• Actively participate in the agency’s care‑coordinating process, including timely reporting of patients’ needs and condition changes, attending patient case conferences, and communicating with the patient, family, physician, other clinicians, agency management staff, case coordinators, and community case managers.
• Receive recognition for performance in an environment that challenges you and provides clear direction and development opportunities.

• Current and unrestricted Licensed Practical Nurse license in the state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of client characteristics.

• LA:
  • Current LPN licensure in LA without restrictions.
  • 1+ year of experience as an LPN.

• Ability to work flexible hours to meet client needs.
• Ability to manage multiple tasks simultaneously.
• Ability to work independently.
• Good communication, writing, and organizational skills.

Pay is based on several factors including local labor markets, education, work experience, certifications, etc. The hourly pay ranges from $21.70 to $32.55 per hour for full‑time employment. Benefits include a comprehensive package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (all subject to eligibility requirements). No minimum wage below applicable laws is applied.
