Responsibilities
  • Monitor the health and well‑being of residents.
  • Provide leadership support and assistance with resident care and services.
  • Assist in training and monitoring of medication care administration.
  • Communicate with resident support services and families.
  • Maintain clinical quality assurance while leading and demonstrating the Mission of Sunrise Senior Living.
  • Identify ongoing health needs and services of residents.
  • Participate in evaluation and documentation for residents with a change in health care services and status.
  • Assist the Resident Care Director with resident evaluations and data gathering at move‑in, 30 days post move‑in, semi‑annually, and with any status changes.
  • Conduct monthly wellness visits for all residents.
  • Follow up with residents who have experienced any fall, accident, or incident and document and report as required.
  • Document all pertinent information in the resident wellness file.
  • Maintain communication with resident families or responsible parties regarding medical needs.
  • Contact resident attending physicians when necessary or upon family request.
  • Obtain weights and vital signs monthly for each resident.
  • Maintain wellness files in accordance with Sunrise policies and regulations.
  • Maintain medical supplies and emergency kits for the community.
  • Provide clinical support and assistance to community team members as needed.
  • Follow infection control practices and promote infection control standards, including resident testing, training, and skills checks.
  • Dispose of blood‑borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials in compliance with universal precautions.
  • Monitor each resident's medication and treatment profile and ensure accurate administration and documentation.
  • Transcribe physician orders and ensure accurate Medication Administration Record/ Treatment Administration Record entries.
  • Demonstrate knowledge of the Six Rights of Medication Pass.
  • Act as liaison for pharmacy services to ensure effective services for residents.
  • Complete medication observation and skill checks for medication care managers at the direction of the RCD.
  • Participate in quality assurance and regulatory compliance activities, including Sunrise Quality Assurance Meetings and Clinical Meetings.
  • Observe resident rights and compliance with all federal, state/provincial, and local regulations.
  • Review physician reports of new residents as directed by the RCD.
  • Order medications and equipment as directed by the RCD.
  • Demonstrate knowledge of Sunrise practices and guidelines in resident care programs for skin, nutrition, behavior, falls, and bowel/bladder care.
  • Assist in implementation, training, and monitoring of documentation related to resident clinical health and wellness.
  • Report resident changes, concerns, and solutions to the RCD as appropriate.
  • Contribute to overall engagement programs and processes, including participation in team member engagement surveys and improvement planning workshops.
  • Maintain compliance with required training and regulatory authorities.
  • Possess effective written and verbal communication skills to teach and guide team members.
Qualifications
  • Graduate of an approved college or school of nursing.
  • Maintain a current state or provincial license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
  • Minimum of one year experience in home health, assisted living, hospital, or long‑term care environment.
  • Demonstrate knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state/provincial, and local regulations.
  • Demonstrate proficiency in computer skills including Microsoft Office (Windows, Outlook, Excel) and Sunrise applications, and ability to learn new applications.
  • Ability to work weekends, evenings, and flexible hours to be available during peak service delivery days and times.
