About the Role:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Onyx Care of Ringgold. As a PRN LPN, you will play a vital role in providing exceptional post-acute rehabilitation care to our patients. Responsibilities:

• Provide skilled nursing care to patients in a post-acute rehabilitation setting, including administering medications, performing vital sign checks, and assisting with activities of daily living (ADLs)

• Develop and implement individualized care plans in collaboration with interdisciplinary team members

• Conduct patient assessments and document findings accurately and thoroughly

• Collaborate with physical, occupational, and speech therapists to ensure comprehensive care

• Communicate effectively with patients, families, and healthcare team members

• Demonstrate a commitment to patient-centered care and a passion for helping patients achieve their rehabilitation goals

Requirements:

• Current LPN license in the state of Louisiana

• 0 minimum years of experience, with a willingness to learn and grow with our team

• Ability to work varied shifts, including weekends and holidays, on a PRN basis
