A rewarding full-time assignment is available for a skilled Registered Nurse with proven home health and OASIS experience. This 13-week contract offers the chance to support patients in their homes and be a vital part of a dedicated, collaborative care team.

• 13-week assignment, with possibility of extension
• Monday through Friday, full-time hours
• On-call responsibilities may apply

• Deliver compassionate, responsive in-home nursing care and skilled interventions
• Accurately assess patients and complete OASIS documentation within required timelines
• Collaborate with coordinating physicians, therapists, and care professionals to achieve optimal patient outcomes
• Perform wound care, medication administration and management, patient and family education, and disease monitoring
• Keep the interdisciplinary team informed of any changes in patient condition and progress

• Active RN license in good standing
• At least 1 year of recent home health RN experience
• Strong OASIS documentation skills are mandatory
• Familiarity with Home Care Home Base EMR (highly preferred)
• Ability to manage a structured daily visit schedule
• Reliable transportation and a valid driver’s license
