Job SummaryOdyssey House Louisiana, Inc. is a seeking a motivated and compassionate contract Nurse Practitioner to work up to 25 hours/week. The Nurse Practitioner provides high-quality Client-focusedhealth care services that meet the assessed and individually planned needs ofall clients in New Orleans Inpatient Services Programs . The Nurse Practitioner will work as part of the multi-disciplinary team (MDT) and is responsible for maintaining standards of nursing care and competency, ethical treatment of patients, understanding of addiction and the medications used to treat substance use disorders, managing symptoms of long term withdrawal with comprehensive triage, consistent documentation on care, review, and implementation of treatment orders, and participation in the multi-disciplinary team via written and verbal communication.
The Nurse Practitioner assumes several key responsibilities in client care and quality assurance.
It is the duty and responsibility of the Nurse Practitioner to:
- Conduct H&P s and intake assessments for inpatient clients;
- Support medical mattersincluding but not limited to medical documentation, and reporting of abuse and neglect;
- Ensure compliance with HIPAA Privacy Rules;
- Participate in monthly One on One meeting and monthly in-services;
- Assist in maintaining theclient census and timely movement of clients through the treatment program;
- Complete incident reports peragency policy, ensure all Incident Reports are submitted to Program Manager and Director promptly;
- Safely administer medications asordered using the five rights and utilizing a MAR;
- Manage patient pharmacy orders bytimely processing PAs, by fax or verbal order, ensure their arrival and logthem onto patient’s MAR;
- Manage theMAR daily, weekly, and monthly as needed;
- Ensure the security of narcotics, keys, and all other medications;
- Participate in medication and medical supply inventory control;
- Educate clients on prescribed medication tapers;
- Ensure communication of medication procedures to discharged clients andscan discharged clients acknowledgement receipts of medications;
- Encourage clients’ participation by empowering them;
- Be flexible when taking care ofclients in need;
- Always maintain professionalism with staff members and clients;
- Complete triage on patient at timeof admission and transcribe it to relevant paperwork for MDT;
- Follow treatment care plans set forth by the medical staff;
- Coordinate/facilitate patient care, work collaboratively and maintain communication with patient careteam (providers, counselors, etc.) to provide effective, timely, and appropriate patient management;
- Provideon-going assessment of acute medical/nursing needs of all residents and refer residents to appropriate treatment centers;
- Coordinate shift changes withnight staff and Medical Staff Daily Inquiries review;
- Check-in with the Front Desk regarding if any clients went to ER after hours and obtain medical feedback sheet and any pertinent labs and notes for the visit;
- Ensure compliance with regulatory bodies including but not limited to CARF, LDH, ASAM;
- Perform other duties as assigned.
Must be able to sit, stand, and walk for long periods up to 40% of the time. Must be able to bend, lift, and twist 10% of the time. May beasked to lift 20 pounds.
WORK ENVIRONMENTWork is normally performed inside of the premises, which isclimate controlled. Risk of exposure to sick patients is consistent with thatwith any clinic setting offering primary care services. The employee will be required to travel to other facilities up to 20% of the time and must bewilling to work various hours to meet operational demands.
MINIMUM QUALIFICATIONSMinimum qualifications include the following:
- Master’s Degree in Nursing Science
- Active, unencumbered LA NP license with prescriptive authority
- Current good standing with regulating National Certifying Board
- At least 1 year of clinical experience
- At least 1 year of Electronic Health Records experience
- CPR certification
- Proficient in MS Office 365
- Excellent communication skills
- Ability to exercise patience, approachability,and consistency in a stress high-stress environment
Preferred qualifications include the following:
- At least 1 year of FQHC and medical home model experience
- Suboxone certification
