Pleasant View Center, a trusted provider of skilled nursing and rehabilitation services in Concord, NH , is seeking Licensed Practical Nurses (LPNs) to join our compassionate and dedicated team. Requirements:Graduate of an accredited LPN/LVN program or nursing degree from an approved institution
Current, unrestricted LPN license in New Hampshire (or eligibility for NH licensure)
Current CPR certification
Experience in long-term care preferred but not required
- Oversee and direct Licensed Nursing Assistants (LNAs) in compliance with facility policies.
- Ensure all nursing care follows regulatory guidelines and Pleasant View Center standards.
- Participate in resident admissions, discharges, and transfers , ensuring smooth transitions and proper documentation.
- Monitor residents for changes in condition , report findings to physicians and families, and document accordingly.
- Transcribe and carry out physician orders.
- Administer medications and perform treatments, including wound care, IV therapy, colostomy care, and tube feedings .
- Coordinate and schedule medical appointments and transportation for residents.
- Maintain resident confidentiality and compliance with HIPAA regulations.
- Assist in the survey process and uphold Pleasant View Center’s high standards of care and safety .
- Foster a collaborative and positive work environment , promoting teamwork across all departments.
