LPN Pediatric Home Care - Part Time

Seeking a Home and Community Based Services Licensed Practical Nurse (LPN) Responsibilities Include

• Providing direct nursing care to acutely or chronically ill patients in their homes, following physician prescriptions and the plan of care.
• Documenting patient visits thoroughly in electronic medical records and complying with regulatory requirements.
• Participating in care coordination, communicating effectively with patients, families, physicians, and team members, and reporting changes in patient conditions promptly.
• Ensuring adherence to infection control procedures and maintaining high-quality clinical service within the scope of practice.

• Current, unrestricted LPN license in the practice state.
• CPR certification, valid driver's license, access to reliable transportation.
• Ability to work independently, manage multiple tasks, and communicate effectively.
• Experience in home care and a minimum of one year as an LPN preferred.
