LPN Peds Home Care - Part-Time Weekend

Role Overview:

The Home and Community Based Services Licensed Practical Nurse (LPN) provides direct nursing care in patients' homes, following physician orders and within the scope of practice defined by state laws.

• Deliver high-quality clinical services, documenting patient visits thoroughly in electronic medical records and ensuring compliance with regulations.
• Coordinate patient care activities, report changes in patient conditions promptly, and communicate effectively with patients, families, healthcare team members, and community resources.
• Follow infection control procedures and adhere to the plan of care established by the supervising Registered Nurse or designated supervisor.

• Current, unrestricted LPN licensure in the practice state.
• CPR certification, valid driver’s license, and reliable transportation.
• Ability to work independently in diverse home environments and manage multiple tasks efficiently.
• Excellent communication, organizational, and documentation skills.

• Home care experience and flexibility to meet client needs.
• Strong organizational skills and the ability to adapt to different situations.
