Registered Nurse - Pediatric Home Care

Role Overview

The Registered Nurse provides comprehensive nursing care in patients' homes, coordinating with healthcare teams to ensure continuity and quality of care.

They assess patient needs, develop and update individualized care plans, and ensure services meet regulatory and payer requirements.

The RN functions independently in diverse home environments, adhering to infection control standards and agency policies.

• Deliver high-quality clinical nursing services within scope of practice
• Coordinate care across multidisciplinary teams from admission to discharge
• Conduct clinical assessments per federal, state, and payer guidelines
• Verify patient eligibility and medical necessity for services
• Create and modify personalized care plans, consulting with physicians and providers
• Ensure care plans meet patient needs with appropriate frequency and duration

• Current and unrestricted RN licensure in practice state
• Valid driver’s license, reliable transportation, and CPR certification
• Ability to adapt to various home settings regardless of client background
• Preferred: at least one year of RN clinical experience, strong communication, organization, and independence skills

Compensation ranges from $32.25 to $48.40 hourly, with benefits including health coverage, incentives, and retirement options. The role emphasizes equitable, patient-centered care and professional growth in a supportive environment.
