Nurse Practitioner - LVAD Coordinator, Heart & Lung Transplant Program East Jefferson General Hospital LCMC Health | Metairie, LA Help Advance Life-Saving Heart Failure and Transplant Care LCMC Health is seeking a compassionate and highly skilled Nurse Practitioner (NP) - LVAD Coordinator to join our growing Heart & Lung Transplant Program. In this advanced practice role, you will serve as a clinical leader and expert resource for patients requiring mechanical circulatory support, working alongside cardiologists, transplant specialists, surgeons, and an interdisciplinary team dedicated to delivering exceptional outcomes.
This is a unique opportunity to directly impact the lives of patients with advanced heart failure through education, care coordination, clinical management, and program development within a leading cardiovascular service line.
- Coordinate and manage care for LVAD candidates and recipients across the continuum of care.
- Assess, diagnose, treat, prescribe, and monitor patients in both clinic and hospital settings.
- Serve as the primary educator for patients and caregivers, ensuring successful adaptation to life with a ventricular assist device.
- Collaborate closely with heart failure and transplant cardiologists, surgeons, nurses, engineers, rehabilitation specialists, and community providers.
- Lead patient care planning, discharge coordination, and long-term follow-up.
- Participate in VAD emergency call coverage and provide expert clinical guidance during urgent situations.
- Support program growth through protocol development, quality initiatives, certification activities, and outcomes analysis.
- Mentor and provide leadership to VAD coordinators and support team members.
- Develop and maintain relationships with referring physicians, EMS agencies, rehabilitation facilities, industry partners, and regional healthcare organizations.
- Participate in community outreach, professional organizations, and ongoing cardiovascular education initiatives.
Minimum Qualifications
- Master's or Doctoral Degree as an Advanced Practice Registered Nurse (APRN) or Physician Assistant.
- Current Louisiana Nurse Practitioner, Clinical Nurse Specialist, or Physician Assistant license with prescriptive authority.
- Current BLS and ACLS certifications.
- 3-5 years of cardiac nursing or advanced practice experience.
- Experience in Heart Failure, LVAD, mechanical circulatory support, transplant, or cardiovascular critical care.
- Board certification in a related specialty.
- Specialty certifications such as: CHFN VAD-C Cardiac/Vascular Nursing (ANCC) CSC HF-Cert CCKE.
- A passion for caring for complex heart failure and transplant patients.
- Strong critical thinking and clinical decision-making abilities.
- Experience managing complex medication regimens and coordinating multidisciplinary care.
- Excellent patient education and caregiver support skills.
- A commitment to quality improvement, evidence-based practice, and program excellence.
- The ability to thrive in a fast-paced environment while balancing multiple priorities.
- Strong analytical, presentation, and relationship-building skills.
