Job Summary:

Alpine Care of St. Charles is seeking a dedicated and experienced MDS Coordinator RN/LPN to join our team in St. Charles, IL. As a crucial member of our healthcare team, you will be responsible for ensuring that our patients receive the highest level of care by coordinating and implementing the Minimum Data Set (MDS) assessment process. • Conduct MDS assessments and reviews to ensure accurate and comprehensive data collection and reporting
• Coordinate with interdisciplinary team members to gather information and complete MDS assessments
• Ensure that all MDS assessments are completed in a timely and thorough manner, meeting all regulatory and accreditation requirements
• Develop and implement effective strategies to improve MDS accuracy and completeness
• Provide education and training to staff on MDS processes and procedures
• Maintain accurate and up-to-date records and reports
• Collaborate with other departments to ensure seamless communication and coordination

• Current RN or LPN licensure in the state of Illinois
• Minimum of 1 year of experience in a healthcare setting, preferably in long-term care or rehabilitation
• Strong understanding of MDS assessment processes and regulations
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment with multiple priorities
• Strong attention to detail and organizational skills
