Job SummaryOdyssey House Louisiana, Inc. is seeking a full‑time Case Manager. The Case Manager will determine eligibility for guests to participate in the agency’s Low Barrier Shelter (LBS). They will assess guests’ needs to create an effective housing plan, meet with guests regularly, ensure they report to referred community resources, use the LA Medicaid system and assist guests with applying and/or recertifying as needed, enter information into Via Link’s Homeless Management Information System (HMIS), document case notes, plan discharges, and monitor programmatic activities.
They may also provide transportation to medical appointments when necessary. The role includes preparing weekly, monthly, quarterly, and annual reports and attending all required department meetings.
- Document services and provide case management to approximately thirty or more guests as assigned.
- Screen, intake, assess, develop individualized housing plans, monitor, link to appropriate community resources, follow up, and facilitate transition to housing.
- Assist guests with obtaining vital records, , driver’s licenses, and other documentation.
- Establish partnerships with housing agencies.
- Assist guests with signing lease agreements when necessary.
- Assess guest insurance status and follow agency policy to maintain Louisiana Medicaid status.
- Enter guest information into the appropriate agency database(s) timely and update as needed.
- Prepare and submit reports to the Project Manager timely.
- Maintain up‑to‑date information on guest occupancy, move‑outs, and vacancies.
- Utilize Motivational Interviewing (MI) to elicit behavior changes.
- Ensure guests are referred to and aware of available vocational, educational, housing, and social services.
- Attend required meetings and trainings.
- Verify results of assistance to close cases per Program Manager approval.
- Provide crisis management when indicated.
- Complete Incident Reports before shift end as required by policy.
- Maintain guests’ licensing requirements.
- Oversee daily guest activities in the LBS.
- Perform other duties as assigned.
- Be over 18 years of age.
- Excellent communication, organizational, and interviewing skills.
- Ability to work with unhoused guests of diverse backgrounds and disabilities.
- B.A. in social work, behavioral science, or three years of relevant experience.
- Valid Louisiana driver’s license with a clean driving record.
- Ability to navigate an eight‑passenger van.
- Personal experience of being unhoused.
- At least three years of case management experience with the target population.
- Database management experience.
