Social Service Counselor 1-2
Lallie Kemp Regional Medical Center is seeking a Social Service Counselor 1-2 to join our dedicated team. Lallie Kemp Regional Medical Center is a 24-bed acute care critical access hospital serving the Florida Parishes region of Southeast Louisiana. We offer a wide range of services including general surgery and a 24-hour emergency department. At Lallie Kemp Regional Medical Center, we are able to bring you a wide range of services close to home, provided by a network of board certified physicians.
This Social Service Counselor 1-2 position is located in the Social Services Department. An ideal candidate should possess the following competencies:

Minimum qualifications include three years of social services experience; or six years of full-time experience in any field; or a bachelor's degree. Experience substitution is available. Positions that provide addictive disorder counseling or prevention services will require certification or eligibility for certification issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor.
Job duties include conducting interviews with patients and families to obtain social history, assessing patients to determine motivation, abilities, job skill potential, and need for financial assistance, transportation, food stamps, in-home aide, and/or any other community resources to aid in discharge planning. The position also involves consulting with attending physicians, the multi-disciplinary team, and other health care professionals regarding community resources, participating on the Multi-Disciplinary Discharge Planning Treatment Team daily, coordinating referrals for families and patients requiring assistance, and assisting patients in medication procurement, as needed.
Position specific details include work location in the Social Services Department, appointment type as probational, and work schedule determined by the Department Manager with on-call availability. Compensation rates are $16.37 per hour for Social Services Counselor 1 and $17.52 per hour for Social Services Counselor 2, with on-call pay included.
