Social Services Director

St Frances Nursing is looking for an experienced energetic Social Services Director.

A Nursing Home Social Services Director manages the facility's social work department, ensuring residents' emotional, social, and psychological needs are met while complying with federal and state regulations. Key responsibilities include conducting assessments, implementing care plans, assisting with discharge planning, and supporting families.

• Resident Care & Advocacy:
  Identify social/emotional needs, conduct psychosocial assessments, and develop care plans.
• Regulatory Compliance:
  Ensure compliance with state and federal regulations regarding resident rights, documentation, and care planning.
• Case Management & Discharge:
  Coordinate discharge planning, assist with placements, and manage referrals to community resources.
• Family & Resident Support:
  Provide counseling, mediate grievances, and facilitate family meetings.
• Department Management:
  Supervise social work staff (social work assistants/designees), conduct staff training, and manage department policies.
• Documentation:
  Maintain accurate, timely, and confidential records of social work interventions.
