Job Overview

The Spanish Speaking Social Worker / Social Services Case Manager assists in the full implementation of the program with a special focus on resolving barriers to engagement in services, ensuring linkage to medical and behavioral health care, with the overall goal of increasing client’s ability to self‑manage, while facilitating integration of care for those individuals and families working with a multiple services systems.

Crescent Care serves approximately 14,000 clients a year. The role includes assessing new and existing clients for program eligibility, biopsychosocial needs, answering and responding to inquiry calls, and providing program information and referrals. The position is fast‑paced and requires close attention to detail. All interactions with and on behalf of clients are documented in the electronic health record. The role collaborates with all members of the treatment and recovery team to maximize integration of care across all systems and programs.

This position reports to the Social Services Supervisor.

Responsibilities
  • Assist in the full implementation of the program with a focus on resolving barriers to engagement in services.
  • Ensure linkage to medical and behavioral health care and promote client self‑management.
  • Assess all new and existing clients for program eligibility and biopsychosocial needs.
  • Answer and respond to inquiry calls and provide program information and referrals.
  • Document all interactions with and on behalf of clients in the electronic health record.
  • Collaborate with the treatment and recovery team to integrate care across all systems and programs.
  • Provide advocacy and linkage to care to increase personal and family stability and achieve recovery goals in mental health, substance abuse, primary care coordination, psychiatric rehabilitation, social recovery support, counseling for barriers, and promote coping skills.
Performance Expectations
  • Provide a minimum of 24 total documented units of service per day.
  • Enjoy working with diverse populations and quickly transition between various types of clientele.
  • Complete progress notes promptly.
  • Ensure expedient client access to resources such as food, housing, health insurance, medical and behavioral health care, substance use treatment, and financial resources.
  • Attend a minimum of 20 hours of continuing education annually.
  • Possess knowledge of and desire to work with a cross‑disability population, including those living with HIV and/or mental health issues.
  • Deliver impactful service to the community as part of a diverse team with new challenges every day.
Requirements
  • Fluent in written and spoken Spanish.
  • Currently licensed social worker (RSW, CSW, LMSW, LCSW) from a program accredited by the Council on Social Work Education; OR licensed nurse (RN or LPN) in Louisiana; OR Bachelor’s Degree (required) or Master’s Degree (preferred) in a human services related field with one year post‑degree experience in direct service to HIV target population; OR Bachelor’s Degree in liberal arts or general studies with at least 16 hours of concentration in relevant fields and one year post‑degree experience in direct service to HIV target population.
  • Demonstrated technical competencies, excellent verbal and written communication skills, ability to complete complex projects, Federal Criminal Background Check, and availability for 40 hours of work per week.
  • Will provide documented proof of highest level of education completed on the first day of employment.
Preferred Qualifications
  • Experience in a Federally Qualified Health Center (FQHC).
  • Knowledge of HIV/AIDS and patient experience.
  • Experience with the LGBTQ community.
