Responsibilities
The senior community health worker (SCHW) for Norton Healthcare plays a vital role in coordinating and overseeing community health initiatives. They provide leadership, training, and guidance to other community health workers ensuring health programs are effective and meet the needs of the patients. Additionally, they act as a liaison between the patient and healthcare providers to advocate for healthcare access and resources.
The SCHW monitors health outcomes to assess the impact of social determinants of health and patient care. Their role is crucial in promoting health equity and improving the overall well-being of the patients they serve.
Required:
- Three years of experience in community outreach/healthcare or a combination of both and a knowledge of serving adults, children and families.
- Certified Technician from the National Child Passenger Safety Certification administration or the ability to obtain within 6 months of hire. KY Certified Community Health worker or the ability to obtain within 6 months of hire.
