Community Living Support Professional

Community Living Support Professional
to assists individual with developmental or intellectual disabilities in their daily living activities within their community. The primary role is to provide support and guidance to help these individuals lead fulfilling and independent lives. The role may involve assisting with personal hygiene, meal preparation, medication management, transportation, household chores, and accessing community resources and services. Community Living Support Professionals often work closely with clients, their families, and other healthcare professionals to create personalized care plans and ensure the well-being and inclusion of their clients within the community.

• Must be 18 years of age
• Must have a high school diploma or its equivalent
• Must have at least one year of experience working with adults with developmental and/or intellectual disabilities
• Must complete all required training as outlined by DDID regulations, including first aid and CPR certification
• Must be able to read, understand and implement written and oral instructions
• Must pass a criminal background check
• Must pass pre-employment and random drug testing
• Must have a valid driver's license, good driving record, and proof of auto insurance
• Must be available to work weekends and holidays

• Assesses skills and interests of clients
• Provides direct care and support to individuals with developmental and intellectual disabilities
• Documents services provided to clients in EVV/OASIS.
• Assists individuals with daily living activities, including personal hygiene, meal preparation, medication administration and transportation.
• Fosters a safe and inclusive environment for individuals to promote independence, socialization and community integration.
• Implements individualized care plans and document individual’s progress.
• Collaborates with families, caregivers and interdisciplinary team to ensure the highest quality of care.
• Promotes and facilitates skill development and personal growth for participants.
• Accompanies participants to community activities, if needed, and facilitate their involvement
• Cooperates and works as a team member with co-workers and supervisors. Treats co-workers and supervisors with respect. Promotes teamwork and maintains a positive attitude.
• Completes initial and annual training-(CPR, First Aid, Tb skin assessment, etc.) as directed by Agency policy and regulatory standards. Attends monthly staff meetings and additional staff development trainings as scheduled.
• Adheres to Agency policy and procedures. Maintains a consistent work attendance record. Limits use of calling off to a minimum and notifies direct supervisor in appropriate amount of time when unable to report to work.
• Fosters collaborative working relationships among members of the leadership and management teams and programs.
