Social Worker; LMSW or LCSW PACE Program

As a Social Worker in our PACE program, you'll play a vital role in helping participants live independently and with dignity in the community they love. You’ll work closely with healthcare professionals to assess needs, develop care plans, and provide supportive services that empower our participants and their families.

• Provide psychosocial assessments and ongoing support to PACE participants and families

• Collaborate with an interdisciplinary care team to create and implement individualized care plans

• Support participants through life transitions, grief, mental health challenges, and social issues

• Assist with community resources, referrals, and advocacy

• Contribute to a culture of compassion, respect, and holistic care

• LMSW or LCSW license (required)

• Experience in geriatric, community health, or long-term care settings preferred

• Excellent communication and problem-solving skills

• A passion for serving older adults and vulnerable populations
