Responsibilities
Since 1985, The Brook Hospitals have been providing quality mental health and addiction treatment services. The Brook Hospitals have two outstanding facilities located in Louisville, Kentucky. The Brook – KMI is a 110‑bed hospital offering a full continuum of inpatient, extended care, residential, partial hospitalization and outpatient services for children, adolescents, and adults.
The Patient Care Navigator of Outpatient Services facilitates inpatient discharges to the next level of care with a focus on transitioning care from the inpatient setting to The Brook Hospitals outpatient programs. The Outpatient Navigator provides groups and individual education to patients on the importance of continuing care, communicates eligibility to key members of senior leadership and associated departments, and routinely collects and enters data, specific to outpatient eligibility, into Midas for trending report purposes.
The Outpatient Navigator will have a thorough understanding of the Step‑Down playbook and implement components of the playbook as needed. A thorough understanding of the inpatient workflow process, as it pertains to case management and discharge planning, is preferred.
Other job functions may include obtaining psycho‑social assessments; conducting family/group therapy on assigned cases; completing documentation per hospital policy and procedure; case coordination; coordinating discharge planning; assisting in developing and maintaining community resource information; performing marketing functions as assigned; assisting in the maintenance of the program environment in compliance with policies of safety and security; ensuring good clinical judgment to ensure optimum patient care;
and participating in continuing education activities.

Master’s Degree from an accredited school in Social Services or Psychiatric.

Must be licensed or under supervision in the state of Kentucky (CSW/LCSW CADC/LCADC, MFTA, LMFT, LPCA, LPCC, LPA, LPP).
