INTRODUCTION: Volunteers of America Mid-States (VOA) is a non-profit organization spanning four states that creates positive change in the lives of individuals and communities through a ministry of service. We provide care and support for individuals with developmental disabilities, healing accountability that brings people together with restorative justice, and free HIV testing and education.

Summary: Service Coordinator will be responsible for ensuring maximum utilization of community resources by residents of VOA housing programs for the elderly or disabled, aimed at maximizing residents’ length of stay and improving quality of life.
Qualifications: This position requires a Bachelor of Social Work or degree in Gerontology, Psychology, or Counseling. A combination of the following skills and knowledge is preferred: aging process, elder services, disability services, eligibility for and procedures of Federal and applicable State entitlement programs, legal issues, drug and alcohol abuse, poverty, and mental health issues. At least two years of experience in social service delivery with senior citizens or non-elderly disabled persons is preferred.
Responsibilities- Complete assessments for residents requesting services.
- Develop case plans based on assessments; update case plans periodically and record progress in case notes after each contact with resident or family members; document all case management activities.
- Identify community resources and develop cooperative relationships with other social service providers.
- Monitor ongoing provision of services and keep providers informed of resident progress as appropriate.
