HOME CARE LPN

Licensed Practical Nurse (LPN) – Home Health

Schedule: Monday–Friday, 8:00 AM–5:00 PM
On-call rotation: one weekday per week and one Saturday per month

License Requirement: Active Kentucky LPN license required (Compact license accepted)
Mileage Reimbursement: TBD

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in the home setting. This role emphasizes patient dignity, safety, and independence while delivering care aligned with individualized plans of care and physician orders.

• Provide nursing care in accordance with each patient's plan of care and physician instructions.

• Monitor vital signs and patient condition; promptly report and document abnormal findings.

• Administer medications, treatments, feedings, oxygen therapy, ostomy care, and other skilled nursing services as ordered.

• Carry out care routines established by interdisciplinary team members, including Physical Therapy, Occupational Therapy, and Speech Therapy.

• Maintain strict infection control standards; identify and report signs of infection or communicable disease.

• Accurately document all nursing interventions, communications, patient progress, and outcomes in compliance with regulatory and organizational standards.

• Order and maintain adequate medical supplies, medications, and nutritional support as needed.

• Ensure all patient needs are met in alignment with the physician's plan of care.

• Monitor, assess, and respond to patient incidents; complete incident reports accurately and notify appropriate personnel.

• Inspect and maintain medical equipment; report malfunctioning equipment to DME providers for repair or replacement.

• Home health experience

• Experience with Homecare Homebase (HCHB) and Point Care

• Wound care and IV therapy experience preferred
