Job SummaryWe are seeking a dual-certified Psychiatric Mental Health Nurse Practitioner (PMHNP) / Family Nurse Practitioner (FNP) to provide psychiatric and medical care across skilled nursing facilities (SNFs) and long-term care settings. This Float Provider role supports geriatric and post-acute populations with complex psychiatric, medical, and neurocognitive conditions.
This is a high-acuity “road warrior” role focused on continuity of care, behavioral stabilization, reduction of hospitalizations, and CMS-compliant clinical excellence across multiple facilities.
Key Responsibilities- Psychiatric and primary care coverage in SNFs and nursing homes
- Management of complex psychiatric, medical, and neurocognitive conditions
- Initial evaluations, follow-ups, medication management, and care planning
- Behavioral and medical crisis response across facilities
- Reduction of hospital transfers and ER utilization
- Interdisciplinary collaboration with facility teams
- CMS-compliant documentation and quality reporting
- Support facility stabilization and consultative needs
- Provider leave coverage
- Staffing shortages and census surges
- New facility onboarding
- Crisis-driven consults
- Active PMHNP certification
- Active FNP or NP certification (dual certification required or strongly preferred)
- Active, clear, and unencumbered Kentucky Nurse Practitioner licensure required
- Active DEA registration required
- Minimum 2 years of experience in psychiatry, geriatric psychiatry, SNF/LTC, or post-acute care settings
- Strong independent clinical decision‑making in high‑acuity environments
- Geriatric psychopharmacology
- Primary care and chronic disease management
- Behavioral crisis stabilization
- Dementia and neurocognitive care
- CMS regulatory compliance
- Interdisciplinary communication and leadership
- Multi‑site clinical practice

- Provides consistent psychiatric and medical leadership across facilities
