GHR Healthcare is seeking a travel nurse RN Med Surg for a travel nursing job in Louisville, Kentucky.

  Mileage reimbursed between visits (from first patient to last patient)
• Weekly Estimated Pay: $2,120 – $2,230

• Active Registered Nurse (RN) license in Kentucky
• Minimum Associate Degree in Nursing (ADN); BSN preferred
• Minimum 1 year of home health nursing experience using HCHB (Homecare Homebase)
• Professional certification required; OASIS certification is a plus
• Strong clinical skills in IV therapy, wound care, and congestive heart failure (CHF) management
• Excellent documentation and time‑management skills for OASIS and electronic charting

• Provide skilled nursing care in patients' homes per physician orders and individualized care plans
• Conduct Start of Care visits, ongoing assessments, and complete accurate clinical documentation in HCHB
• Complete timely OASIS assessments and meet productivity expectations (30 points per week)
• Coordinate interdisciplinary care with case managers, therapists, and primary care providers to optimize outcomes
• Perform IV therapy, wound assessments and treatments, medication management, and CHF monitoring with a focus on patient education
• Deliver compassionate, patient‑centered care that supports independence and safe transitions from hospital to home
