Rehab LPN/LVN who excels in this role:

• Assist the RN with initial assessments and patient reassessments per policy.
• Identify and implement appropriate rehabilitation nursing interventions.
• Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
• Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
• Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
• Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.

Applicants should have a current Ohio LPN or LVN license and graduate of an accredited Licensed Practical/Vocational Nurse program. Additional requirements include:

• Certifications: BLS required; IV certification with 6 months of hire required.
• Experience: 6+ months LPN/LVN in Medical/Surgical preferred
