Provides basic nursing functions and services to individuals and families under moderate supervision in a clinic, school or community setting. Applies technical, interpersonal, analytical, and organizational skills to health problems, and conducts communicable and preventive disease case‑finding investigations to prevent and control diseases and promote population health.
Responsibilities- Conduct nursing assessments using CCSG service guidelines and develop and implement plans of care based on identified health risks.
- Provide basic nursing care consistent with CCSG, Nursing Scope and Standards of Practice, and agency policies, procedures and licensing laws.
- Document services delivered according to CCSG and Administrative Reference standards; maintain confidentiality and privacy.
- Apply problem‑solving strategies and participate in specialized or program‑specific clinics.
- Develop and conduct educational counseling and teaching activities for clients and families; serve as a coordinator of a specific program when needed.
- Assess the health status of individuals, groups and populations through data analysis; identify at‑risk populations and advocate for them.
- Assist in providing nursing services to high‑risk or limited‑access populations; mentor new staff in agency policies, practices and procedures.
- Perform work with increasing independence and may mentor LHN‑I nurses.
- Conduct communicable and preventative disease case‑finding investigations to prevent and control diseases and promote population health.
- Registered Nurse (RN) license in Kentucky or a compact state.
- One year of RN experience or a Bachelor of Science in Nursing (BSN) qualification may substitute for the experience requirement.
- Maintain current RN licensure in Kentucky or a compact state, completing required annual updates and any additional credentials within a reasonable time.
