We are seeking an experienced Travel Registered Nurse (RN) for a Home Health/Hospice assignment serving patients throughout Louisville, Kentucky, and surrounding coverage areas. This position is responsible for providing comprehensive in-home nursing care, conducting OASIS assessments, and managing a diverse patient population with varying healthcare needs.

The ideal candidate will have strong home health experience, proficiency with OASIS documentation, and experience using Homecare Homebase (HCHB).

Location: Louisville, KY

Schedule: Monday–Wednesday, 8:00 AM – 4:30 PM

Employment Type: Temp for 1 year

Pay Rate: $55.92/hr

Key Responsibilities
  • Conduct Start of Care (SOC), Resumption of Care (ROC), Recertification, and Agency Discharge visits.
  • Complete accurate and timely OASIS assessments and documentation.
  • Deliver skilled nursing care to home health and hospice patients in their homes.
  • Assess patient conditions and develop individualized plans of care.
  • Monitor patient progress and communicate changes to the interdisciplinary care team.
  • Educate patients and caregivers on disease management, medications, treatments, and preventive care.
  • Maintain compliance with agency, state, and federal regulations.
  • Document all patient interactions and clinical findings in Homecare Homebase (HCHB).
Required Qualifications
  • Active Kentucky Registered Nurse (RN) License.
  • Graduate of an accredited nursing program.
  • Minimum of one year of Home Health experience.
  • Minimum of one year of OASIS experience.
  • Experience using Homecare Homebase (HCHB).
  • Valid driver's license, reliable transportation, and auto insurance.
  • Ability to travel throughout the assigned coverage territory.
Preferred Qualifications
  • Previous travel nursing experience.
  • Strong assessment and critical thinking skills.
  • Excellent communication and patient education abilities.
  • Experience managing a high-volume home health caseload.
Coverage Area

Primary coverage includes ZIP codes: 40214, 40215, 40216, 40258, 40272, 40118. Additional travel within Metro Louisville may be required.
