We are seeking an experienced Travel Registered Nurse (RN) for a Home Health/Hospice assignment serving patients throughout Louisville, Kentucky, and surrounding coverage areas. This position is responsible for providing comprehensive in-home nursing care, conducting OASIS assessments, and managing a diverse patient population with varying healthcare needs.
The ideal candidate will have strong home health experience, proficiency with OASIS documentation, and experience using Homecare Homebase (HCHB).
Location: Louisville, KY
Schedule: Monday–Wednesday, 8:00 AM – 4:30 PM
Employment Type: Temp for 1 year
Pay Rate: $55.92/hr
Key Responsibilities- Conduct Start of Care (SOC), Resumption of Care (ROC), Recertification, and Agency Discharge visits.
- Complete accurate and timely OASIS assessments and documentation.
- Deliver skilled nursing care to home health and hospice patients in their homes.
- Assess patient conditions and develop individualized plans of care.
- Monitor patient progress and communicate changes to the interdisciplinary care team.
- Educate patients and caregivers on disease management, medications, treatments, and preventive care.
- Maintain compliance with agency, state, and federal regulations.
- Document all patient interactions and clinical findings in Homecare Homebase (HCHB).
- Active Kentucky Registered Nurse (RN) License.
- Graduate of an accredited nursing program.
- Minimum of one year of Home Health experience.
- Minimum of one year of OASIS experience.
- Experience using Homecare Homebase (HCHB).
- Valid driver's license, reliable transportation, and auto insurance.
- Ability to travel throughout the assigned coverage territory.
- Previous travel nursing experience.
- Strong assessment and critical thinking skills.
- Excellent communication and patient education abilities.
- Experience managing a high-volume home health caseload.
Primary coverage includes ZIP codes: 40214, 40215, 40216, 40258, 40272, 40118. Additional travel within Metro Louisville may be required.
