Job Description
The Registered Nurse (RN) in Home Infusion provides high-quality, compassionate care to Home infusion patients. The RN is responsible for assessing patient needs, developing individualized care plans, and delivering direct nursing care in accordance with the physician’s orders. The RN utilizes physicians, community resources, and the patient’s support system ensuring that all care delivered meets agency standards and regulatory requirements.
Responsibilities- Conduct thorough assessments of patients' health status, including vital signs, response to therapy, and overall progress.
- Administer medications, treatments, and procedures as prescribed by physicians.
- Monitor patients' progress and document appropriately.
- Identify and report any adverse reactions or complications to supervising physician and pharmacist.
- Educate patients and their families on infusion procedures, medication administration, and how to manage potential side effects.
- Provide guidance on maintaining a sterile environment and proper use of infusion equipment.
- Collaborate with the healthcare team, including physicians, pharmacists, and case managers, to develop and implement individualized care plans.
- Adhere to all agency policies, procedures, and regulatory requirements, including infection control and patient safety standards.
- Participate in quality improvement initiatives and contribute to the development of best practices for home health care.
- Meet established productivity standards by managing caseloads efficiently.
- Ensure timely and accurate submission of patient care notes, reports, and other required documentation to meet agency and regulatory deadlines.
- Balance patient care needs with productivity goals, ensuring high-quality care without compromising efficiency.
- Manage and maintain infusion supplies, equipment, and medications, ensuring that all are available and properly stored.
- Coordinate with the pharmacist and technicians to replenish inventory and patient supplies as needed.
- Participate in liaison cross‑training and on-call rotation.
- Associate's degree in nursing, BSN preferred.
- Graduate of an accredited nursing program.
- 1 year of job-related experience.
- Experience with central lines, PICC lines, ports, and peripheral IVs.
- Must have a valid Kentucky RN license.
- Must obtain BLS certification within 90 days of hire.
- Valid driver's license.
- Regular and reliable attendance; healthcare workers must be present and punctual for all scheduled shifts.
