Position Overview
The Registered Nurse provides comprehensive home health nursing care to patients across the Metro Louisville area. This role is responsible for completing OASIS assessments, including start of care, recertifications, resumption of care, and discharge visits, while ensuring high-quality, patient-centered outcomes. The RN will collaborate with the care team and maintain accurate documentation in accordance with agency and regulatory standards.
Schedule & Work Details- Schedule:
Monday through Wednesday, 8:00 AM to 4:30 PM - Weekly Hours Guaranteed: 36 hours
- Pay Rate: $51 to $53 per hour
- Productivity Expectation: 30 points per week
- Coverage Area: 40214, 40215, 40216, 40258, 40272, 40118, and surrounding Metro Louisville areas
- On-Call Requirement:
One weekend per month for SOC visits and on-call coverage - Mileage Reimbursement:
Yes, between patient visits
- Conduct OASIS assessments including Start of Care, Recertification, Resumption of Care, and Discharge visits
- Deliver skilled nursing care to home health patients with a variety of medical needs
- Develop, implement, and evaluate individualized patient care plans
- Accurately document patient care and assessments within Homecare Homebase (HCHB)
- Educate patients and families on disease management, medications, and care plans
- Coordinate care with physicians, therapists, and interdisciplinary team members
- Ensure compliance with all clinical, regulatory, and agency standards
- Travel between patient homes within designated coverage areas
- Active Kentucky RN License
- Minimum of one year of OASIS experience
- Experience with Homecare Homebase (HCHB)
- Strong clinical assessment and documentation skills
- Ability to work independently and manage a travel-based caseload
- Previous travel nursing experience in home health
- Strong time management and organizational skills
- Excellent communication and patient education abilities
- Flexibility and adaptability in a dynamic care environment
