Deliver essential care in the comfort of your patients’ homes as an experienced and dedicated Registered Nurse specializing in home health services. This contract position offers the opportunity to make a significant impact on patient outcomes, leveraging your clinical expertise and compassionate communication skills. Ideal candidates are organized, autonomous professionals who thrive in a dynamic, field-based environment while ensuring exceptional standards of care.
Schedule- Monday Friday, 8:00 AM 5:00 PM
- Complete comprehensive OASIS assessments and develop individualized care plans.
- Deliver direct skilled nursing services such as wound care, medication administration, IV therapy, and chronic illness management.
- Monitor patient progress and communicate notable changes in condition to physicians and care teams.
- Educate patients and families regarding disease management, treatment options, medications, and preventive care.
- Accurately document all interventions and communications within Home Care Home Base (HCHB).
- Coordinate effectively with therapists, social workers, and home health aides to ensure seamless care.
- Participate in case conferences, staff meetings, and continue professional education.
- Adhere to agency policies, regulatory standards, and home health best practices.
- Active RN license in good standing within the state of practice.
- Minimum one year of recent field-based home health nursing experience.
- Proficient with Home Care Home Base (HCHB) and OASIS.
- Strong clinical assessment, problem-solving, and organizational skills.
- Excellent verbal and written communication.
- Reliable transportation, current drivers license, and auto insurance.
- BSN and experience managing large home health caseloads are preferred.
- Familiarity with Medicare guidelines and home health regulatory compliance a plus.
