Job Overview
Work Monday - Friday (no weekends).
Job DescriptionThe intent of this job description is to provide a representative summary of the major duties and responsibilities performed by incumbents of this job.
Responsibilities- Administer and oversee self‑administered and prescribed medications per established schedules as ordered.
- Monitor clients' vital signs and other physical health information, and maintain accurate records reflecting the data and treatment results.
- Administer injectable medications, ensuring accurate prescribed dosage and timely delivery; educate clients on management of side effects and obtain preauthorization of prescriptions/medications when needed.
- Complete and maintain accurate documentation relating to provision of care, medications, other client‑related services, and advocacy, including staff notes, case reviews, letters, reports, etc.
- Maintain client medical record/charts related to admissions and discharges.
- Transcribe medical provider orders and obtain requested medical information pertaining to clients.
- Administer pregnancy tests and drug screens on clients and document results as needed.
- Maintain accurate inventory counts of all medicines and medical supplies, and ensure adequate replacements are ordered in a timely manner.
- Schedule outside medical appointments for clients as ordered by a medical provider.
- Monitor medical equipment and testing per CLIA standards.
- Participate on a team with MD/APRN/RN to manage the flow of medication management clinics.
- Up to eighteen months of education or training beyond the high school level in nursing or a related field of study.
- License for Practical Nursing in the Commonwealth of Kentucky.
- Graduation of an accredited program.
- Minimum 6–12 months of nursing experience.
- Occasional exposure to threat of bodily harm from clients and their families.
- Limited routine exposure to blood, body fluids, or tissue.
- Reliable transportation required for frequent travel between work sites.
- Exercise principle‑centered leadership and customer service responsiveness.
- Follow safety management policies and Joint Commission standards.
- Maintain accurate record keeping and inventory management.
