Center Well Home Health is seeking an LPN/LVN to provide one-on-one nursing care in patients’ homes, under RN supervision. You will implement care plans, educate patients and families, and maintain detailed records while coordinating with the broader health team.
You must hold a current nursing license for the practicing state, have a valid driver’s license with insurance, and possess CPR certification along with at least two years of clinical LPN/LVN experience.
