Clinic Nurse – RN
Are you an RN who thrives in a dynamic, community-based care setting and finds purpose in advocating for medically complex children? Home of the Innocents is seeking a Clinic Nurse – RN to serve as the medical liaison between our Kosair for Kids Complex Care Center, parents and guardians, and specialty physicians and practitioners. In this role, you'll provide expert clinical care on- and off-site, lead coordination of complex medical appointments, and provide functional direction to the Clinic Nurse – LPN and Certified Medical Assistants.
A Day in the Life
The Clinic Nurse – RN plans, coordinates, and executes on- and off-site medical appointments and clinics under the direction of the Director of Support Services. You'll collaborate with physicians, subspecialty providers, the interdisciplinary care team, and neighborhood nurse managers, providing direct resident care within RN scope of practice and ensuring continuity of care that meets or exceeds federal, state, local, and facility standards.
Schedule:
Primarily day shifts within a 24/7 care environment. Full-time, Monday - Friday, 8:30 AM - 5:00 PM. Hours may fluctuate around resident appointments.
How This Position Contributes to Our Mission
- Plan, coordinate, and execute on- and off-site medical appointments and clinics per physician orders
- Perform and document nursing care within RN scope, including medication administration, ostomy management, enteral nutrition, wound care, tracheostomy/airway management, central line management, and ventilatory support monitoring
- Collaborate with physicians, subspecialty providers, and the interdisciplinary team to maintain continuity of care and communicate resident changes
- Coordinate transportation and accompany residents to medical appointments, providing direct care as medically indicated
- Engage and support parents and guardians in appointments and shared decision-making
- Provide functional direction to the Clinic Nurse – LPN and Certified Medical Assistants; maintain timely, accurate EHR documentation of all visits and communications
Skills and Qualifications
Required:
- Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN); unencumbered RN license in Kentucky
- Minimum one (1) year of direct care RN experience in acute or long-term care
- Cardiopulmonary and/or ventilator/tracheostomy care experience
- Current CPR/BLS certification (provided if needed)
- Valid driver's license, clear driving record, and ability to drive multi-passenger vehicles and secure wheelchairs for resident transport
Preferred:
- Experience caring for children with complex medical needs
- Pediatric and/or long-term care experience
- Experience with palliative and end-of-life care
- Experience supporting individuals with developmental and intellectual disabilities
- Bilingual or multilingual communication skills
