Assessment and Field Support Nurse (Registered Nurse / RN)
The Registered Nurse / RN role is responsible for skilled and non-skilled assessments, in addition to providing oversight and support for Bright Star Care patients receiving caregiving, home health and private duty care services. Reporting directly to the Director of Nursing (DON), this role serves as a clinical resource for patients, families, caregivers, and nursing staff by ensuring quality care, regulatory compliance, and continuity of services.
The role conducts initial assessments and ongoing reassessments, supports the admission process, provides clinical education and competency validation, and partners with the Director of Nursing on quality improvement initiatives. This position is highly visible in the field, serving as a liaison between patients and families, field staff, and the Director of Nursing to ensure exceptional clinical care and patient satisfaction.
This position is not responsible for employee performance management, disciplinary actions, scheduling, or direct supervision of staff. Instead, the role focuses on non-skilled and skill assessments, education, support, and quality.
Essential Responsibilities
Clinical Oversight
Complete comprehensive initial assessments for new patients receiving non-skilled and skilled home care services.Perform scheduled reassessments and supervisory visits in accordance with agency policy, regulatory guidelines, Joint Commission recommendations, physician orders, and/or payer requirements.Develop, review, and update individualized plans of care and task lists.Identify changes in patient condition and communicate recommendations to the Director of Nursing, Director of Operations, and other members of the care team.Ensure services are delivered safely and in accordance with clinical standards and best practice, Bright Star Care policies, and Joint Commission recommendations.Patient & Family Support
Assist with the admission process and provide a smooth transition for new patients and families into Bright Star Care services.Educate patients and families regarding care plans, expectations, safety, and available resources.Serve as a trusted clinical resource for patients, families and field staff.Collaborate with the Director of Nursing and Director of Operations to resolve issues and promote positive patient outcomes.Caregiver & Nursing Support
Provide support to caregivers and nursing staff in the field.Conduct caregiver skills validation and new hire competency check-offs.Identify educational opportunities and provide coaching, mentoring, and training.Support ongoing clinical competency and best practices through field visits and one-on-one education.Serve as a resource for caregivers and nurses requiring assistance with patient care concerns.Quality & Compliance
Participate in quality assurance and performance improvement initiatives.Assist with clinical chart audits to ensure documentation accuracy, completeness, and regulatory compliance.Support agency readiness for Joint Commission accreditation and regulatory surveys.Monitor compliance with agency policies, infection prevention standards, and documentation requirements.Collaborate with the Director of Nursing and Director of Operations to identify process improvements that enhance patient care and clinical outcomes.Collaboration
Communicate patient status updates and concerns promptly to the Director of Nursing and Director of Operations.Foster positive working relationships among patients, families, caregivers, nurses, physicians, and community partners.Act as a clinical liaison between field staff, patients/families, and agency leadership.Qualifications
Current unrestricted Registered Nurse (RN) license in the state of practice.Minimum of two years of nursing experience.
