Job Description Summary

UofL Health is seeking a dedicated, highly skilled APRN to provide comprehensive primary and post-acute care to residents within our long-term care and assisted living facilities. The successful candidate will manage a caseload of geriatric patients presenting with complex, multi-system chronic conditions and transitional post-acute recovery needs. This individual will collaborate closely with an interdisciplinary medical team to deliver high-quality, evidence-based care aimed at optimizing functional status and patient comfort.

Key Responsibilities
  • Conduct comprehensive geriatric evaluations, admission physicals, and annual wellness exams.
  • Diagnose and manage acute status changes, episodic illness, and complex chronic conditions.
  • Formulate, implement, and monitor individualized post-acute rehabilitation and transitional care plans.
  • Prescribe medications, order diagnostic imaging, and interpret laboratory findings.
  • Coordinate wound care treatments and monitor therapeutic rehabilitation milestones.
  • Facilitate proactive updates and care conferences with residents, families, and legally authorized representatives.
  • Provide clinical guidance, mentorship, and telephone triage support to facility nursing staff.
  • Partner with the Medical Director, attending physicians, therapists, and social workers.
  • Complete accurate, timely, and compliant point-of-care EMR documentation.
  • Palliative and end-of-life care as needed.
  • Lead advance care planning conversations and clarify patient goals of care with families.
  • Establish customized pain and symptom management protocols for residents with terminal illness.
  • Provide emotional and holistic support to residents and families navigating end-of-life transitions.
  • Collaborate closely with community hospice agencies to ensure cohesive continuity of comfort care.
Required Qualifications
  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
  • Active, unrestricted Kentucky Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) license.
  • Board-certified as an Adult-Gerontology Primary Care NP (AGPCNP), Adult-Gerontology Acute Care NP (AGACNP). Family NP (FNP) may be considered with previous experience in palliative or geriatric care.
  • Valid DEA registration and state Controlled Substance Registration (CSR).
  • Current Basic Life Support (BLS) certification.
Experience And Competencies
  • Minimum of two years of geriatric care with autonomous clinical experience managing older adults.
  • Proven track record in skilled nursing facilities (SNF) or long-term care (LTC) environments.
  • Advanced diagnostic acumen regarding polypharmacy, dementia behaviors, and frailty syndromes preferred.
  • Dedicated formal palliative care or hospice care also preferred.
