Description:

The Registered Nurse implements professional nursing practice centered on the patient as a whole person.

The focus is on creating a caring environment that promotes healing and wellness.

Nursing practices are based on evidence and current research, as well as departmental policies and procedures.

Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity.

Nursing excellence is promoted through relationships among nursing staff and across disciplines.

Responsibilities include coordinating, ensuring, and providing direct patient care activities on the unit.

This includes provision of nursing rounds, assessments, and treatments.

The nurse facilitates admission, transfer, and discharge activities.

Patient and family-centered education is also part of the role.

The nurse is responsible for care coordination and individualized treatment planning as applicable.

The role requires comprehensive assessment of patient needs in accordance with nursing scope of practice.

Cultural, age, and developmental considerations are taken into account during assessments.

Risk factors and presenting complaints are identified and documented.

Objective and subjective data is collected from patients and, when necessary, their families.

Initial admission assessments by other healthcare team members are verified.

A problem list is developed, prioritizing patient needs based on input from the inter-professional team.

Actions and interventions relevant to the patient's culture and age are designed and implemented.

Desired outcomes are determined in coordination with patients and families according to the medical plan of care.

The nurse fully implements patient care therapies and procedures within established policies and professional guidelines.

Procedures, treatments, and processes are explained to patients and families as needed.

Initiatives are taken to alleviate patient stress and discomfort.

Changes in patient condition are recognized, responded to, and communicated appropriately.

Ongoing assessment and responses to procedures and therapies are documented according to hospital policy.

Tasks are delegated appropriately, and team members are overseen in patient care activities.

Delegated patient care is monitored for accuracy and completeness in the implementation of the plan of care.

Educational needs of patients are identified and addressed based on their level of comprehension.

The discharge process is facilitated by executing discharge orders in a timely manner.

Input into patient/nurse assignments is provided, considering staffing, unit activity, and patient acuity.

Active participation in unit-based activities that support organizational values is expected.

A positive group dynamic and healing environment are promoted through words and actions.

Nursing excellence is assured in accordance with accepted standards and evidence-based practice.
