Position: Housing Case Manager & Social Services Coordinator

Housing Case Manager & Social Services Coordinator Duties and Responsibilities
  • Assists clients in achieving their housing goals
  • Willingness to work with clients with serious mental illness
  • Develops a person-centered service plan for each client
  • Assesses each client's needs, with an emphasis on strengths and barriers related to getting & keeping their housing
  • Works with property management staff to help clients maintain their housing
  • Identifies strategies that will help clients maintain their housing and strengthen their recovery
  • Coordinates/facilitates speakers, groups or events that foster community, identify helpful resources, and reinforce wellbeing
  • Supports clients in understanding the basic expectations of being a tenant, lease requirements, household maintenance, etc.
  • Collaborates with landlords and other community partners, advocating for client needs and rights.
  • Assists clients in applying for rental assistance, SNAP benefits, social security income, etc.
  • Participates in regular supervision and team meetings
