St. Vincent de Paul Louisville (SVDP) is dedicated to supporting individuals in need, including those experiencing homelessness, poverty, addiction, or mental health challenges. The organization provides essential services such as food, shelter, affordable housing, counseling, and financial assistance, all aimed at promoting self-sufficiency. SVDP operates a range of facilities, including emergency shelters, supportive housing, thrift stores, and an enrichment center for at-risk youth.

Through a comprehensive approach, SVDP addresses the root causes of chronic poverty and homelessness, empowering individuals and families to achieve stability and a brighter future. The organization serves up to 600 individuals across its campus and scattered-site housing programs.

Role Description

This full-time, on-site Case Manager position is based in Louisville, KY. The Case Manager will support individuals by assessing their needs, creating personalized service plans, and connecting them to available community resources. Responsibilities include providing advocacy, assisting with housing and financial stability planning, monitoring client progress, and maintaining accurate documentation. Collaboration with community partners and participation in program development initiatives will also be key components of this role.

Qualifications

  • Experience in case management, client advocacy, or social services
  • Strong assessment and planning skills, including the ability to create and manage individualized service plans
  • Excellent interpersonal, communication, and relationship-building skills
  • Proficiency in record-keeping, documentation, and administrative tasks
  • Knowledge of community resources, homelessness, addiction, and mental health issues
  • Ability to work collaboratively within a team and with external partners
  • Cultural competency and a commitment to diversity and inclusivity
  • Bachelor’s degree in Social Work, Human Services, or a related field preferred
  • Familiarity with the social and economic challenges faced by underprivileged populations
  • Proficiency with computer and data entry skills is a plus
