The Targeted Case Management Coordinator is responsible for coordinating and providing Targeted Case Management (TCM) services to residents participating in the RISE Campus Independent Living Program. This position assists young adults in developing and achieving goals related to housing stability, education, employment, healthcare access, financial literacy, and independent living skills. The Coordinator serves as a primary advocate and resource navigator, ensuring residents receive individualized support and access to community-based services that promote long‑term self‑sufficiency.
The Targeted Case Management Coordinator maintains compliance with all applicable regulations, documentation requirements, and agency policies while fostering a trauma‑informed, strengths‑based, and culturally responsive environment.
Essential Duties and Responsibilities Targeted Case Management Services- Conduct assessments to identify resident strengths, needs, barriers, and service goals.
- Develop, implement, and monitor individualized service plans.
- Coordinate and facilitate access to community resources, benefits, healthcare, education, employment, housing, and supportive services.
- Maintain regular contact with residents to monitor progress and address emerging needs.
- Advocate on behalf of residents to ensure access to appropriate services and supports.
- Coordinate transitions into and out of the Independent Living Program.
- Assist residents with developing independent living skills, including budgeting, household management, employment readiness, time management, and self‑advocacy.
- Support residents in establishing and maintaining stable housing and employment.
- Provide coaching, encouragement, and accountability related to personal and program goals.
- Facilitate life skills groups, workshops, and educational activities as assigned.
- Complete all assessments, service plans, progress notes, referrals, and case documentation within established timelines.
- Maintain accurate and confidential resident records.
- Ensure compliance with Medicaid Targeted Case Management requirements, agency policies, and applicable regulatory standards.
- Participate in audits, quality assurance reviews, and program evaluations.
- Collect and report program outcome data as required.
- Collaborate with internal staff, service providers, schools, employers, healthcare professionals, and community agencies to coordinate services.
- Develop and maintain relationships with community partners that support resident success.
- Participate in multidisciplinary team meetings and case consultations.
- Assist with program outreach and resource development efforts.
- Respond appropriately to resident crises and behavioral health concerns.
- Assist with crisis intervention, de‑escalation, and safety planning when needed.
- Follow all agency safety procedures, incident reporting requirements, and emergency protocols.
- Promote a safe, supportive, and respectful living environment for all residents.
- Attend required trainings, meetings, and professional development activities.
- Support program initiatives, special projects, and agency events.
- Perform other duties as assigned to support program operations and resident success.
- Bachelor’s degree in Social Work, Human Services, Psychology, Counseling, Sociology, or a related field required.
- Master’s degree preferred.
- Minimum of two (2) years of experience in case management, independent living services, housing services, behavioral health, social services, or a related field preferred.
- Experience working with youth, young adults, or vulnerable populations preferred.
- Knowledge of Targeted Case Management principles and community resource coordination.
