Community Outreach Coordinator

Essential Functions:

Assists in organizing, scheduling, and coordinating community outreach in partnership with other health, social service, and community agencies. Educates the community on topics related to Medications for Opioid Use Disorder (MOUD) and community reentry to reduce stigma and foster acceptance. Builds community partnerships with other Opioid Use Disorder (OUD) providers, housing programs, and social service providers for potential referrals of clients upon release or discharge from MORE center.

Assists with reentry needs of client to prepare for release to ensure they have transportation to treatment, housing, and other social service needs.

Examples of Work: UNDER ADMINISTRATIVE SUPERVISION, meets with referred clients to assess and refer clients to identify available financial, medical, social, and community assistance and services. Communicates and meets frequently with treatment team to coordinate services to prepare clients for release. Seeks assistance from Certified Alcohol and Drug Counselor (CDAC) or LMDC staff when a need arises that is beyond scope of work to communicate the clients' needs.

Follows up with clients post release to ensure smooth transition back to society and continued treatment. Attends community meetings and events to provide information about services and maintain positive public relations. Receives calls, schedules, and meets with clients, as needed, post release to provide additional support for reentry. Disseminates clinic paperwork to clients for completion and assists them with filling out forms if needed for intake, housing, etc.

Collects statistical data, prepares reports on activities and operations, reviews reports to insure timeliness, accountability and compliance with established medical protocols and quality assurance guidelines, and completes special projects. Documents in-client electronic health record regarding interactions and plans post release. Assists counselors and peer support specialist with case management efforts for clients. Performs related work.

  • Meets regular and punctual attendance standards and any special needs of the position as determined by management, including mandatory overtime and performing related work
  • Travels to various locations

Minimum Requirements:

  • Associate's in Business Administration, Computer Sciences, Criminal Justice, Liberal Arts, Office Administration, Public Administration, Public Health, Social Science, or related field
  • One year of records management, customer service, or community outreach experience
  • Equivalency:
    An equivalent combination of education and experience may be substituted

Minimum Requirements:

GENERAL Must provide one's own source of transportation Must possess and maintain a valid driver's license Must work a varied schedule Must be accessible by phone and able to return calls from clients on weekends and holidays as required Must meet other requirements as dictated by funding source

Must complete an annual tuberculosis evaluation Must be vaccinated for hepatitis B, have provided evidence of immunity, or have signed a declination form within ten days of initial assignment Must wear personal protective equipment as mandated by OSHA

Special Requirements:

Must obtain KYConnector Training within one year of employment

Experience in working with Medications for Opioid Use Disorder (MOUD) programs and clients
