Social Service Worker
Under direction, performs social service work. Responsible for assisting the homeless in obtaining primary health care and a more stable living environment. Performs related work as required.
Examples Of The WorkConducts on-site visits to areas frequented by homeless persons to identify those in need of health care services. Interviews client to determine their health care needs. Develops individual client service plans. Makes referrals, accompanies clients, and/or provides transportation to service providers. Monitors the delivery and outcome of services provided to clients by medical facilities, social service agencies, and community resources. Provides ongoing intensive case management.
Assists clients in obtaining medical and financial entitlements and benefits. Helps clients utilize community resources, develop independent living skills, and become reintegrated into the community. Educates clients on good health habits and available services to assist with health needs.
Bachelor's degree in social work or a closely related field. One year of experience working with persons in extreme poverty or the homeless. An equivalency of training may be substituted as determined applicable by Civil Service. Verification of education must be provided before eligibility list goes into effect.
Physical Requirements And Working ConditionsLifts and/or carries small children weighing up to 20 lbs. Spends prolonged periods of time standing and walking over various types of inner-city terrain.
