Full-time Case Manager I for our High Fidelity Wraparound program in Louisville, Kentucky. As a Case Manager
, you will be responsible for providing in‑home support services for families whose children are at risk of removal. This position will be based in Louisville but will be mainly working in the field.

High Fidelity Wraparound (Wraparound) is an evidence-based approach rated as “Promising” on the Title IV‑E Prevention Clearinghouse. Wraparound centers on individualized care planning and coordination to support children and families with complex needs. The core principles of Wraparound include family voice and choice, natural supports, team‑based collaboration, community‑based services, individualized planning, strength‑based approaches, persistence, outcome‑based and cultural competence. Ensuring staff are adequately trained, the program is sustainable, and there are adequate resources to support the needs of youth and families enrolled in the Wraparound program is essential to success.

We are a team of 500 employees across seven states providing services such as Family Preservation & Reunification, Behavioral Health, Therapeutic Foster Care, Adoption Services and Older Youth Services to help families and children build resilience and thrive.
Responsibilities- Work independently and create your own schedule based on your caseload and availability of your assigned families.
- Provide in‑home, direct services to families and children in your caseload. Caseloads typically consist of 10‑12 families seen weekly.
- Create documentation for all services in our Electronic Record System.
- Participate in rotating on‑call schedule with team members (approximately one week every two months).
- Develop and maintain consultative and liaison relationships with DCBS and other members of the youth’s treatment team (biological parents, school, etc.).
- Collaborate with outside parties including family supports, relatives, schools, courts, hospitals, doctors, etc.
- Travel to families’ homes in and around Louisville – mileage reimbursement is included. Hours outside the office are estimated at 60%, including travel and time conducting home visits.
- Set schedule in collaboration with the team. Standard hours are about 9 am‑6 pm, though you may need to work until 8 pm or later when required.

Qualifications- Bachelor’s degree in social work, counseling, psychology, sociology, child & family studies, or another approved related field of human services.
- Two years of relevant experience in social work or adoption (preferred).
- Valid driver’s license, acceptable driving record, and auto insurance coverage to meet travel requirements; may require transporting youth if needed.
- Current certification in First Aid/CPR and de‑escalation training.
